• you have been notified of an adverse outcome about a patient
• you have been contacted by an attorney requesting your medical records

CLAIMS HISTORY REQUESTS

Requests from insureds: No charge
Please send a written request that is signed by the insured. The request must include the insured’s full name, address, and phone number. The Policy Number should be provided if this information is available. Submit information to appropriate party below.

Requests from third parties: $20.00, mailed or $30.00, faxed
Third parties are asked to submit a request in writing along with a written authorization from the insured to release the information. They must include a check made out to Professional Risk Management Services, Inc., or credit card number with an expiration date and the amount to charge to the card to process the fee. Submit information to appropriate party below.
